Crash reports - EPay

Thank you for choosing to request and pay for your crash report using E-Pay. A service fee will be added to the statutorily set fee of $5.00 for crash reports obtained through this service. For credit card transactions, that service fee will be 2.35% with a $1.00 minimum . If you do not wish to incur this fee, or have entered this site in error, please go to directions on how to obtain your report by mail.

In order to obtain a copy of a crash report completed by the ISP, first verify the ISP handled the traffic crash. To determine if ISP handled the traffic crash, refer to the "Investigated By" box located in the upper left corner of the motorist report. If this box does not indicate "Illinois State Police", please contact the listed investigating agency to obtain a copy of your traffic crash report. If you do not have a motorist report, contact the ISP Patrol Records Unit at: 217/785-0614, to verify if the ISP handled the traffic crash.

The Illinois State Police rarely handles traffic crashes occurring on municipal streets.

Please Note:
All requests for crash reports will be governed by the Illinois Freedom of Information Act (FOIA), 5 ILCS 140/1, and you will be receiving a REDACTED VERSION of the requested records.

Ordering your crash report using E-Pay will not return your crash report automatically. Each request is processed in the order which it was received and can not be e-mailed or mailed until the report has been approved. For this reason, your request may take 10 or more business days to process.

Also, if your e-mail account has a spam or junk mail filter, your e-mailed response may go there. Please review your spam or junk mail file for any e-mail message from e-mail address ISP.CrashReport@illinois.gov or a subject line of "Illinois State Police Traffic Crash Report".

The following information will assist ISP in locating your crash report quickly. The fields listed in red below, as well as those noted with an asterisk on the next page, are required. Provide as much information as possible.

  • The Agency Crash Report Number - located in the upper middle portion of the motorist report.
  • A driver's first and last name.
  • The date of the crash.
  • The location of the crash; example: Interstate 55, State Route 29 or U.S. Route 50.
  • County of occurrence.

PLEASE NOTE: While a shopping cart is available to request multiple crash reports, please limit the number of reports requested to thirty or less. If more than thirty reports are requested at once, the transaction will fail and all crash reports will have to be resubmitted.
